CCSA WAIVER AGREEMENT

CANCELLATION & RESCHEDULING POLICY

Appointments may be canceled or rescheduled without penalty up to 48 hours before the scheduled appointment time. Cancellations or rescheduling requests made less than 48 hours before the scheduled appointment may be subject to a cancellation fee.


MAKEUP CLASSES

Makeup classes are offered only for classes a student misses, subject to availability and facility scheduling. Makeup classes must be scheduled in advance and are not guaranteed. Makeup classes have no cash value and may not be transferred or exchanged for credits or refunds.


NO REFUNDS FOR MISSED CLASSES

No refunds, credits, or prorated tuition will be provided for missed classes, regardless of the reason for the absence. Families are encouraged to take advantage of available makeup classes whenever eligible.


FORCE MAJEURE

If classes or appointments must be canceled, postponed, or modified due to circumstances beyond the facility's reasonable control—including, but not limited to, severe weather, natural disasters, government orders, public health emergencies, utility outages, or other force majeure events—the facility will make reasonable efforts to reschedule affected classes or provide makeup opportunities when feasible. Refunds will not be issued for cancellations resulting from force majeure events.


GROUP APPOINTMENT POLICY

A single student may book a group appointment timeslot and invite additional students to attend, up to the maximum permitted enrollment. The student who books the appointment is solely responsible for all fees associated with the reservation. The facility will not split appointment fees among multiple accounts. Any cost-sharing arrangements must be handled privately by the participating families.


DROP PROCEDURE:

PARENTS MUST NOTIFY THE SCHOOL TO DROP A STUDENT FROM CLASS. Only a written notice via email, regular postal mail or hand delivered to our front desk will be acceptable.


Please note: You are responsible for payment for your student's classes WHETHER OR NOT YOUR STUDENT ATTENDS CLASS until the time you notify the staff VIA WRITTEN NOTICE. Please do not rely on your student to verbally let us know that he/she will no longer be attending classes. If a student stops coming to class without notification then that student's account will be charged for the additional 30 days. This charge will be for holding the student's place in that class instead of offering that place to one of the many on a waiting list.


BILLING AUTH


I represent and warrant that if I am purchasing something or paying for a service from this facility or from other merchants through this facility that (i) any credit card or bank account draft (ACH Draft) information I supply is true and complete, (ii) charges incurred by me will be honored by my credit card company or financial institution, and (iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.


I hereby authorize (if online payment is made or autopay information is provided) this facility to charge my ACH draft, or credit card account. I understand that a 30 day written notice is required to terminate billing and I am responsible for payment whether or not my student attends classes until I notify this facility in writing to drop my student from class(es).


Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services.


RULES

WHAT TO WEAR
Boys or Girls may wear tucked in T-shirts and shorts OR Leotards for Girls. NO chewing gum or dangling jewelry. Hair should be pulled neatly and securely away from the face so that is stays up for the entire workout. Girls should not wear bows or other large hair ornaments that may cause discomfort during activity. All students should have activity-appropriate footwear during class. Personal items should be left in cubby holes. Jewelry should not be worn during classes. PLEASE LEAVE JEWELRY ARTICLES AT HOME. This facility's staff will not be responsible for ANY items that may be lost or stolen. Be sure your student's personal items are marked with their name.


ARRIVAL AND PICKUP
Be sure your student arrives 5 minutes before (no earlier please) his/her scheduled class time. Please pick up your student on time. Please inform us if you know you will be late picking up your student. Instruct your student to wait inside the building and you should escort them from the building to your car. During peak times the parking lot is crowded. Please take into consideration that our students may include young children. Please drive slowly and carefully. Do not take a chance on your student running to and from your car.


Camp Payment Policy

Payment for each individual camp must be paid in full no later than 30 days prior to the camp start date. A deposit is required at the time of registration to reserve a spot. Deposits are non-refundable, but will be retained as a credit on the family account if a student withdraws.

Failure to pay the remaining balance by the due date may result in the forfeiture of the student’s spot, particularly if a waitlist exists or the camp reaches capacity.


WAIVER

As legal guardian of my designated student(s) (student(s)), I hereby consent to all student(s) participating in this facility's program(s). I recognize that potentially severe injuries can occur in any activity involving height or motion, including tumbling and related activities including cheerleading, tumble tramp, trampoline, stunting, pyramids, dance, swimming, martial arts, gymnastics and physical activity in general. I understand that it is the express intent of all staff and personnel to provide for the safety and protection of my student(s) and, in consideration for allowing my student(s) to use these facilities, I hereby COVENANT NOT TO SUE and FOREVER RELEASE this facility, affiliated and partner companies and organizations, property owners and lessors, staff, contractors, subcontractors, teachers, coaches, owners, directors and other members involved in this facility's program(s), from all liability and for any and all damages and injuries suffered by my student(s) during instruction, supervision, and/or control during any and all classes or extra activities.


This Waiver and Release constitutes the entire agreement between the Parties with respect to the matters addressed herein and supersedes all prior or contemporaneous discussions, representations, understandings, or agreements, whether oral or written. The Participant and, if applicable, the Parent/Legal Guardian acknowledge that they are not relying on any statement, promise, representation, or assurance not expressly set forth in this Waiver and Release.


Medical Conditions, Allergies, and Special Accommodations

The Participant and, if the Participant is a minor, the Parent/Legal Guardian represent and warrant that they have disclosed to the Organization all known allergies, medical conditions, disabilities, physical limitations, behavioral concerns, medications, or other circumstances that may affect the Participant's safe participation or that the Organization should reasonably be aware of.


The Parent/Legal Guardian further acknowledges that they are responsible for taking all reasonable steps in advance to manage or accommodate any such condition, allergy, disability, or limitation unless the Organization has expressly agreed in writing to provide specific accommodations. Any agreed-upon accommodation must be documented in writing, and the Parent/Legal Guardian agrees to provide any necessary medications, equipment, supplies, instructions, or other materials required to safely implement the accommodation. Failure to disclose relevant information or provide required materials may result in the Participant being denied or removed from participation if the Organization determines that safe participation cannot be reasonably ensured.

AI POLICY USAGE

Purpose


Revival Productions and Chester County School of the Arts recognize that artificial intelligence (AI) can be a useful tool for administration, marketing, education, and creative development. We support responsible use of AI while protecting human creativity, artistic integrity, privacy, intellectual property, and individual rights.

Our guiding principle is simple:


AI may assist human creativity and productivity, but it does not replace human judgment, authorship, or responsibility.


Appropriate Use


AI may be used as an assistive tool for purposes such as:

  • Brainstorming and idea development
  • Drafting or editing non-confidential communications
  • Administrative organization and planning
  • Marketing and social media brainstorming
  • Lesson planning and educational activities
  • Research and summarization
  • Translation and accessibility
  • Preliminary creative concepts for productions


All AI-generated material must be reviewed by a human before it is used publicly, professionally, or educationally.


Privacy and Confidential Information


Confidential or personally identifiable information must never be entered into public or unapproved AI systems.


This includes student records, grades, medical or disability information, contact information, financial information, personnel records, private communications, confidential casting information, and other sensitive organizational information.


Users are responsible for understanding how an AI tool collects, stores, and uses information before submitting organizational or student-related material.


Student Names for Spelling and Grammar


For routine spelling, grammar, proofreading, formatting, and editing purposes, a student's name may be entered into an approved AI tool when necessary to ensure the student's name is spelled or formatted correctly.


This limited use is permitted only when:


  • The student's name is necessary for the spelling, grammar, formatting, or proofreading task.
  • No additional confidential or sensitive student information is included.
  • The information is not being used to generate a profile, evaluation, assessment, or other substantive information about the student.
  • The AI tool being used is approved for organizational or educational use and has appropriate privacy protections.


Users should provide the minimum information necessary to complete the task. A student's name should not be combined with sensitive information such as grades, medical or disability information, contact information, disciplinary information, or confidential student records.


Students, Artists, Images, and Voices


AI may not be used to create, clone, manipulate, or distribute a student's, employee's, artist's, or other individual's voice, likeness, image, or performance without appropriate permission.


This includes:

  • Voice cloning
  • Deepfakes
  • Synthetic performances
  • AI-generated images presented as real people
  • Making someone appear to say or perform something they did not


The Organizations will not use AI to misrepresent an individual's participation, abilities, endorsement, or performance.


Minors, Performance, and PIAA-Affiliated Students


Because many of our programs involve minors participating in dance, theatre, music, and other performing arts, AI must be used in a manner that protects students' privacy, accurately represents their abilities and participation, and respects applicable school, athletic, and organizational requirements.


AI may not be used to create, substantially alter, or distribute content involving a minor in a way that:

  • Misrepresents a student's actual singing, dancing, acting, musical, or other performance abilities.
  • Creates or alters an audition, rehearsal, competition, recital, concert, or theatrical performance in a way that makes the student appear to perform something they did not actually perform.
  • Creates a false impression that a student participated in a production, performance, competition, recital, concert, or other event when they did not.
  • Alters a student's appearance, voice, body, movement, or performance without appropriate permission.
  • Creates or distributes AI-generated images, videos, audio, or performances that could reasonably be mistaken for an authentic recording of the student.
  • Uses a student's name, image, likeness, voice, or performance without appropriate permission.
  • Falsifies awards, credits, résumés, biographies, casting information, performance history, or other credentials.
  • Could negatively affect a student's school standing, eligibility, participation, or compliance with applicable school, athletic, or organizational requirements.


PIAA-Affiliated Students


Some students participating in our performing arts programs may also participate in PIAA-sanctioned school athletics. AI-generated or AI-modified content involving these students must not misrepresent their athletic or performing arts participation, accomplishments, abilities, or eligibility.


Where a student's performing arts participation intersects with school athletics, users must comply with all applicable PIAA, school district, school, and organizational rules regarding student participation, media, photography, video, name and likeness, and other representations of students.


This policy does not replace or supersede PIAA rules or school district policies.

If there is uncertainty regarding whether AI-generated or AI-modified content could affect a student's eligibility, participation, privacy, or representation, the content should be reviewed by an appropriate administrator before it is created, published, or distributed.


Parental/Guardian Permission


For minors, appropriate parental or guardian permission must be obtained when required before using AI to create, modify, or distribute identifiable content involving a student's image, voice, likeness, or performance.


AI should never be used to make a minor appear to say, sing, dance, act, play an instrument, or otherwise perform something that they did not actually do without clear disclosure and appropriate authorization.


Auditions and Casting


Audition materials must accurately represent the performer's abilities.


AI may not be used to substantially alter or generate an audition performance in a way that misrepresents the performer's actual singing, acting, dancing, or other skills.


Reasonable technical editing, such as trimming or correcting recording issues, is permitted unless otherwise specified by the audition requirements.


Artistic Integrity and Copyright


The Organizations value human artists and will prioritize human creative work whenever reasonably practical.


AI-generated material must not be assumed to be free of copyright or other intellectual property restrictions. Users are responsible for respecting copyright, licensing agreements, trademarks, and the rights of other artists.


For significant creative work, users may be asked to disclose material AI involvement.


Human Oversight


AI may assist with decisions, but it may not independently make final decisions regarding:

  • Students
  • Grades or academic evaluations
  • Casting
  • Hiring
  • Discipline
  • Employment
  • Artistic or educational placement

A qualified human must review and make these decisions.


Prohibited Uses


AI may not be used to:

  • Expose confidential student or organizational information
  • Impersonate another person
  • Create unauthorized deepfakes or voice clones
  • Falsify auditions, credentials, or professional work
  • Harass, discriminate against, or harm another person
  • Misrepresent AI-generated work as human-created when disclosure is required
  • Violate copyright, contracts, licensing agreements, or other applicable rules


Reporting Concerns


Students, parents/guardians, employees, artists, and other community members should report suspected misuse of AI to an instructor, supervisor, administrator, or Executive Artistic Director.

Violations may result in appropriate corrective or disciplinary action based on the circumstances.


Our Commitment


Revival Productions and Chester County School of the Arts will continue to evaluate AI as technology evolves.


We encourage our community to approach AI with curiosity, creativity, honesty, responsibility, and respect for human artists and learners.


When in doubt: use AI as a tool—not a substitute for the human being doing the work.ation if the Organization determines that safe participation cannot be reasonably ensured.
